Provides pre-admission screenings for placement in a nursing facility other long-term care setting. CARES provides assessment and review for long term care services and will make referrals for needed services. CARES will assess clients in the home, hospital or extended care setting to establish a level of care and recommend the setting that is most appropriate. An application to Medicaid is advisable in conjunction with or prior to the request for a CARES assessment.
